À l'aide de milnacipran en collaboration avec la doxépine peut augmenter le risque d'une affection rare mais grave appelée syndrome sérotoninergique, qui peuvent inclure des symptômes tels que la confusion, les hallucinations, crises, des changements extrêmes de la pression artérielle, augmentation du rythme cardiaque, fièvre, transpiration excessive, des tremblements ou des tremblements, une vision floue, des spasmes musculaires ou de la rigidité, des tremblements, troubles de la coordination, de l'estomac, des crampes, des nausées, des vomissements et de la diarrhée. SURVEILLER de PRÈS: l'utilisation Concomitante d'agents à activité sérotoninergique tels que les inhibiteurs de la recapture de la sérotonine, les inhibiteurs de la monoamine-oxydase, les antidépresseurs tricycliques, les récepteurs 5-HT1 agonistes, les alcaloïdes de l'ergot, le lithium, le millepertuis, phénylpiridine opioïdes, le dextrométhorphane, et le tryptophane peut potentialiser le risque de syndrome sérotoninergique, qui est une complication rare, mais grave et potentiellement mortelle condition considéré comme le résultat de l'hyperstimulation du tronc cérébral 5-HT1A et 2A des récepteurs. D'autres autorités recommandent un intervalle de 1 semaine entre la fin du traitement avec le milnacipran et le début du traitement avec un 5-hydroxytryptamine (récepteurs 5HT-1) agoniste (par exemple, le sumatriptan).
